Consultant Surgeon, Wexham Park Hospital.                                                          General and laparoscopic surgery, and paediatric surgery, BCSP endoscopist, advanced polypectomy, Frimley Health NHS Foundation Trust, 16th May, 2012 on-going.  One in ten on-call commitment as well as elective work/outpatients. 10.5 PAs. The on call covers various acute admissions including general surgical and colo-rectal and paediatric patients. I have done therapeutic endoscopic upper GI bleeding, managed wide range of surgical emergencies conservative and operative as required. I did wide range of operations perforated peptic ulcers, small and large bowel obstructions, stomas, splenectomise for trauma, Hartman’s procedures, etc; Elective operations including open/laparoscopic inguinal, femoral hernia repair, incisional hernias, formation/ reversal of ileostomies/colostomies open and laparoscopic, bowel resections, colonic and anterior resections, reversal of Hartman’s, laparoscopic and open cholecystectomies with or without cholangiogram, lymph node biopsies, endoscopies, colonoscopies, etc. I am the lead for paediatric surgery in our hospital networking with colleagues across the region. Attending colo-rectal and upper GI MDTs. I am a certified bowel cancer screening endoscopist and do regular BCSP lists in our trust.

CLINICAL EXPERIENCE

 

 

Current post: One in ten, general surgical on call Rota. I have covered elective as well as emergency for general, upper GI and Colo-rectal surgery. I have done laparoscopic and open emergency procedures, emergency cholecystectomies, oncall for upper GI bleeding. Laparotomies for different colo-rectal emergencies, including small and large bowel obstructions, Hartman’s, perforated and bleeding peptic ulcers. I did elective and emergency colo-rectal cancer operations, including right, left hemicolectomies, sigmoid colectomy and anterior resections, attended and active member of the colo-rectal MDT meeting, upper GI MDTs, I do independent endoscopy lists. I have managed abdominal and thoracic traumas. Elective laparoscopic cholecystectomies, TEP and TAP inguinal and femoral hernia repairs. I managed various surgical patients and liaised with different specialties when needed to provide the best patient outcome.

Recently I have been doing elective pediatric surgery including different types of hernias, circumcisions, PPV and EUAs.

 

 

Previous post: I was working as a registrar and then senior clinical fellow in colorectal, upper GI, vascular and endocrine. I did emergency and elective duties covered upper GI and laparoscopic surgery as well as colo-rectal surgery. Operating – independently, under supervision, assisting and training as appropriate. (laparotomies for small and large bowel obstruction, perforated and bleeding duodenal ulcers, colostomies, right, sigmoid colectomies, strangulated inguinal, Para-umbilical and femoral hernias, appendicectomies, Hartman’s procedure, etc. I have assisted and contributed in anterior resections and abdomino-perineal excisions. I did Laparoscopic procedures like cholecystectomies, inguinal hernias (TEP) and assisted with Laparoscopic Nissen’s fundoplications, excision of gastric GIST and splenectomies, etc.

I did end-arterectomies, assisted in EVARS (exposure and deployment), and open aortic aneurysm repairs, fem-fem crossovers, fem pop and fem distal bypasses. I have done below and above knee amputations. I have done skin grafts as well. I did thyroidectomies, parathyroidectomies, laparoscopic splenectomies as well as thoracoscopic and laparoscopic sympathectomies. I did high number of special colo-rectal and upper GI laparoscopic and open procedures.

 

Out-patient clinics – managing new (including the two-week rule) and follow up patients independently.

 Daily ward rounds, supervision of junior surgical staff.

Active participation in clinical audit, clinical governance and research.

Teaching of the medical students and surgical trainees.

 

Previous Posts

 

As an SHO I was doing 1/6 on call rota, managing emergency and elective admissions. I gained special experience in the pre-operative assessment of patients undergoing various surgical procedures and post-operative care including patients in Intensive and High Dependency Units. I assisted my consultant in the operating theatre in doing major operations laparoscopic and open. I performed some operations like excision of sebaceous cysts, lipomas, appendicectomies, open hernias and varicose veins under supervision. I was doing out patient clinics and presenting audits.

 

Overseas Training

 

The training was in The Medical City, Baghdad University, which is a big campus of different specialities. I worked under the supervision of 5 consultants in General Surgery, and 6 consultant surgeons in Trauma and Orthopaedics, Neurosurgery, Urinary, Vascular and Thoracic, Plastic, and Paediatric Surgery. The training programme included: -

 

First Year: With Mr Hisham Al-Doori, FRCS, Consultant Surgeon, as an SHO in general surgery for 12 months, looking after the inpatient, doing outpatient clinics, assisting him in the theatre in doing major operations (elective and emergency), I did some minor operations like hernias, appendicectomies, varicose veins, haemorrhoids, hydroceles. I attended regularly the weekly grand round in the general surgical department, and audits as well as journal clubs.

I passed the primary examination for Basic Sciences in General Surgery (Arabic and Iraqi Board).

 

Second Year: With Mr Dhia’a Al-Kattan, FRCS, Consultant Orthopaedic and Trauma Surgeon, as an SHO for 12 months, I did outpatient clinics, assisted him in the theatre, and did close reductions and external fixation to simple fractures.

 

Third Year: with Mr M AlAni, FRCS, Consultant Urologist, as an  SHO for 4 months.             

 Paediatric Surgery for 2 months.        Plastic Surgery for 2 months. Thoracic & Vascular Surgery for 2 months with Prof. N AlHasani, FRCS, Consultant Thoracic Surgeon.

 

Neurosurgery for 2 months with Prof. H Al-Khalili, FRCS, Consultant Neurosurgeon.

 

As a registrar in general surgery in Medical City Centre in Iraq, I had good chance to learn most of the general surgery operations like hernias, GI and endocrine surgery. My duties included the management of emergency and elective cases.

Education, Training and Supervision

 

Recently appointed as the University of Oxford Medical Students Lead in Surgery

 

I am the clerkship director for AUC (American University of the Caribbean) medical school for general surgery since 2016.

 

I have been heavily involved with training, teaching and clinical supervision to medical students, doctors and nurses.

 

I am a colonoscopy trainer accredited by JAG.

 

I am a faculty member of the BSSC running for the last three years in our hospital.

 

I was a faculty member in Oxford School of Surgery Core Surgical Training Program for our hospital, 2013.

 

I have been clinical and educational supervisor to FY1, FY2, CT1 and CT2, SCF and SPRs for the last 12 years.

 

I have been the interim surgical tutor for the Oxford Deanery for Surgical Training between January 2018-September 2018 and was a panel member in the ARCP.

 

I am the clerkship director for the American University of the Caribbean and RUSM medical students for six years started on the 7th December, 2015. I organized their surgical rotation, regular teaching and simulation session.

 

I participate in the regular teaching Rota for our FY1, FY2, CT1&2.

 

 

I do regularly formal assessments for medical students, doctors at FY1,2, CT, HST and have the necessary training. I participate actively in the Local LFG meetings.

 

I was the interim surgical tutor from February- September 2018. I was a member in the ARCP panel for Oxford Deanery for CT1&2 on the 6th June, 2018.

AUDITS

1. Audit on my personal OGD/ colonoscopy use of sedation/ completion rate and complications April,2012-March, 2013. This is to look into whether the practice is with the guidelines including the use of sedation mainly over 70s, completion rate and polyp detection rate.

 

1. Audit on the Indications and the outcome of the endoscopic investigation of the Iron deficiency anaemia, March, 2007, St Helier Hospital. To see whether our trust referral policy is adhering with the referring guidelines and what is the percentage of referrals who really are iron deficiency anaemic.

 

2. Audited the two weeks referral compliance for the colo-rectal cancer, Aug, 2006, Epsom General Hospital, to look into whether we are compliant with the national guidelines of doing the investigations within the time frame.

 

3. Audit on the outcome of the day case laparoscopic hernia repairs, March, 2006, Epsom General Hospital. To look into the practice of doing day surgery laparoscopic inguinal hernia repair as a day procedure, including patient satisfaction questionnaires and whether it was safe practice to improve the service.

 

4. Audit on the use of the drains in the breast conserving surgery for early Ca breast, June, 2004, Lincoln County Hospital. Looked into the changing practice of our firm of not leaving drains in breast conservation procedures, looking into the complication rates of infection, seroma/ haematoma formation and adopting this policy as a safe practice.

 

5. Audit on the use of antibiotics in the elective laparoscopic cholecystectomy and re-audit closing the loop on 23/02/2015, 19/01/2016.

 

6. Audit on the use of the extended thrombo-prophylaxis in emergency colo-rectal operations.

 

7. Audit on the compliance of antibiotic prescription of the inpatients.

 

8.Audit on paediatric circumcision part of regional paediatric KPIs comparing our figures to the national average.

 

9. Audit on the compliance of antibiotic prescription of the inpatients

 

Currently doing a project on the benefit of using the outpatients MRCP scan  in patients with resolving abnormal LFTs prior to cholecystectomy.

Management and Leadership

 

I was managing and organising the surgical rota for the registrars and CT/ FY1&2.

 

I was responsible for managing the two weeks rule colo-rectal cancer referral triage and clinic allocations.

 

I am a member ( surgical ) of the Major Trauma Group, 2016-2018.

 

I was the lead for our Ambulatory Care Unit for the 3 years. I have made change to increase the number of patients coming to the service. I trained an HCA to become the Ambulatory Service Coordinator. This has made our work flow smoother and improved patients experience.

 

I am the Infection Control Lead for the surgical department since 2016.

 

I am with one of my colleagues responsible for shortlisting, interviewing and appointing junior and senior clinical fellows to our surgical department for the last three years.

Consolidate my experience in general surgery. Advance courses in laparoscopy, advanced trauma management, therapeutic endoscopy courses. I want to be more involved in teaching, mainly medical education for medical students and junior doctors training.
